Providing a little extra support to empower people to live independently at home
Our team supports people to live in their own independent homes within the community. In these settings, we continue to focus on people's needs and developing their strengths.
Our Supported Living Service empowers people to live in their own homes, supported by a team of healthcare professionals, family and friends. Through the development of a bespoke Support Plan, this service helps each person to develop and reach their full potential.
Our team supports people with activities of daily living, personal care, cooking, light housework, social activities and community engagement, all guided by the person's needs and strengths.
Our Supported Living Service provides a continuum of care and support under the Embrace umbrella. Some of the people we support enter through this service and over time their needs may progress towards a suitable community home under our Residential Support Services. Similarly, it is our aim that many people will enter through Residential Support Services and progress into an independent community setting under Supported Living Services. This dual approach ensures that the needs and goals of all people can be met by our team, ensuring a seamless transition from one setting to another.
